The E-MECHANIC Study: A Comprehensive Look at Diet vs Exercise

Published in the American Journal of Clinical Nutrition, the E-MECHANIC (Exercise Metabolic Across Regimes Converging With Healthy Activity and Nutrition Interests in Cardiometabolic Early Risk) trial examined the effects of diet versus aerobic exercise training on body composition and metabolic health markers.

Researchers at Virginia Tech divided 120 older adults with elevated cardiovascular disease risk into three groups for a 5-month intervention:

1) Aerobic exercise only

2) Dash diet (low sodium, rich in fruits/veggies) only

3) Combination of aerobic exercise + Dash diet

The results were eye-opening:

- The diet-only group lost nearly twice as much weight (13.8 lbs) compared to exercise-only (7.2 lbs)

- Only the combination and diet groups saw decreases in body fat percentage

- All groups experienced reductions in blood pressure, but the diet groups fared best

- Diet and combination groups showed bigger improvements in cholesterol and triglyceride levels

As the researchers summarized, "Diet had profound effects on all outcome measures...the combination of diet and exercise was essential for most optimal weight loss and metabolic benefits."
